POSITION SUMMARY

The Patient Care Coordinator (PCC) serves as the dedicated liaison between our ambulance service and our hospital partners. This role ensures seamless communication, efficient scheduling of patient transfers, and consistent fulfillment of our contracted partnership. The PCC is primarily focused on logistics, customer service, relationship management, and service coordination--not clinical care. This position may have varied locations based on assignment and placement in contracted facilities.


DUTIES & RESPONSIBILITIES

  1. Serves as administrative resource and liaison between Patriot Ambulance Service and its hospital and facility partners.
  2. Maintains "big picture" view of operational activities and patient care across the ambulance service based on patient acuity, and admission, discharge, and transfer activities and facilitates the effective transport scheduling with Dispatch.  
  3. Receive, prioritize, and schedule all transportation requests from hospital or facility partners.
  4. Communicates effectively and tactfully with all staff and management both internally and externally with contracted facilities, hospitals, other external agencies, patients and their families.
  5. Works directly with case managers, unit secretaries, nurses, and hospital administration.
  6. Monitors transfer timelines, identify delays, and communicate updates to hospital and/or facility staff.
  7. Displays a courteous and professional attitude.
  8. Maintains a neat, clean, and professional appearance per company Dress Code Policy.
  9. Troubleshoots issues related to patient flow, discharge timing, or transport availability.
  10. Maintains performance metrics, reporting trends to leadership and hospital partners.
  11. Act as the on-site representative for customer satisfaction, operational feedback, and relationship-building.
  12. Collect and prioritize the medical information regarding patients and the type of transportation needed, relay information to Dispatch and any other required parties. Collect and submit any and all patient information including, but not limited to, patient demographics and insurance, Physician Certification Statements, hospital face sheets, and other pertinent patient information necessary for billing and/or state or federal requirements.
  13. Represent Patriot Ambulance Service at meetings, as required or requested, and relays information to management and staff.
  14. Prepares and submits various reports as directed or requested.
  15. Must have strong language skills. Must have the ability to properly answer, evaluate, and process hospital and/or facility requests.
  16. Must have strong operational knowledge of computers and software applications. The use of computer-based electronic Patient Care Report (PCR) software is required.
  17. Represents the integrity, philosophy, and company values, including company policies and procedures, in a positive manner both internally and externally.
  18. Collaborates with other departments and their leaders by implementing and/or following workflow processes.
  19. Maintains professionalism when working with clinical staff, physicians, and administrators.
  20. Other duties as requested or required.


JOB RESPONSIBILITIES RELATED TO PATIENT PRIVACY (HIPAA)

  1. The employee is expected to protect the privacy of all patient information in accordance with the company's privacy policies, procedures, and practices, as required by federal (and state) law, and in accordance with general principles of professionalism as a health care provider. Failure to comply with the company's policies and procedures on patient privacy may result in disciplinary action up to and including termination of employment or of membership or association with Patriot Ambulance Service.
  2. The employee may access protected health information and other patient information only to the extent that is necessary to complete their job duties. The employee may only share such information with those who have a need to know specific patient information necessary to complete their job responsibilities related to treatment, payment, or other company operations.
  3. The employee is encouraged and expected to report, without the threat of retaliation, and concerns, incidents, or issues regarding the company's policies and procedures on patient privacy and any observed practices in violation of that policy to the designated Privacy Officer.
  4. The employee is expected to actively participate in mandatory company privacy training and is required to communicate privacy policy information to coworkers, students, patients, and other in accordance with company policy.


QUALIFICATIONS

  1. High School Diploma, Home School Equivalency, or General Education Diploma (GED). Associate's Degree or higher is preferred.
  2. Proficiency in computer systems and software applications.  
  3. Valid State of Michigan Driver's license and approved driving status based on acceptable Motor Vehicle Record and insurance requirements.
  4. Minimum of 1 year experience in healthcare is required. Previous health care coordination, medical office operations, dispatch, patient flow experience, sales, or other related experience is preferred.
  5. Background check must be clear of a felony conviction.
  6. Excellent critical thinking, organizational, interpersonal relationship, and verbal and written communication skills.
  7. Must be customer service-focused and provide professional support to staff.
  8. Ability to prioritize multiple requests in a fast-paced environment.
  9. Must be able to prioritize, think clearly and quickly, and use sound professional judgement.
  10. Must possess strong skills in conflict resolution and rational decision-making.
  11. Must possess the ability to perform all duties in a courteous, positive, and professional attitude.
  12. Current employees must be in good standing.
